Day Services

701 Water Street West
This location is the administration building and offices of the leadership team. Day services such as day respite and personal care is offered in this building, increasingly for persons with multiple disabilities. The program objective with clients supported from this building is to access the community for various development opportunities, leisure and recreation, and other related activities. The furniture refinishing department and upholstery services are also located in this building (from entrance off Notre Dame Street).

 

340 Court Street
This location is the Employment Outreach Program (EOP). The entrance to the EOP (lower level) is off Cedar Street. The objective of the EOP is to provide employment services to clients with a primary focus on skills training and access to employment. The emphasis is on meaningful supported employment and independent employment in the community.


Residential Services

 

287 Winter Street
This location is a group home that offers five permanent beds; one accessible bedroom on the first floor and four bedrooms on the second floor. There is also one respite care bed offered on the third level of this home. Residents of this home attend day services Monday to Friday. There is staff supervision to the residents Monday to Friday from 3:00 pm to 8:00 am and 24-hour supervision on the weekends.

 

420 Highland Avenue

This location is a home that offers three individual affordable and accessible apartment units. Each apartment unit is a self-contained two-bedroom apartment with full kitchen, dining, living room space, and with an accessible bathroom and laundry facility.

One apartment unit is for persons with disabilities that possess a high level of independent living skills.

One apartment unit is an accessible respite care apartment. Families are responsible for the respite care provider. Families can request respite care at this location for their adult children with disabilities by scheduling time for respite care.

One apartment provides housing to two persons that are dependent on a staff supported model of housing care.

 

422 Highland Ave
This location is a home that offers two small option apartments. One is a three-bedroom apartment with staff supervision to residents from 3:00 pm to 8:00 am on weekdays and 24 hour supervision on the weekends. One is a two-bedroom apartment with 24 hour supervision daily.

 

405 Granville Street
This location is an apartment building with six apartments connected with a common area. Four of the apartment units are two-bedroom supported apartments. Two of the apartment units are three-bedroom small options with 24 hour staff supervision. The common area serves as a social connection to all residents and, for most residents, the supper meal is provided in the common room. There is one staff position in the common area through the weekday evenings and part of the weekends to assist residents in the supported apartments. Residents who reside in the supported apartments are responsible for the care of their apartment and preparation of their breakfast and lunch meals.

 

395 MacEwen Rd
This location is an apartment building with seven apartments connected with a common area. Six of the seven apartments are supported apartments; there are three one-bedroom apartments and 3 two-bedroom apartments. There in one three bedroom apartment with staff supervision 3:00 pm to 8:00 am on the weekdays and 24 hour supervision on the weekends. The common area serves as a social connection to all residents and, for most residents, the supper meal is provided in the common area. Residents who reside in the supported apartments are responsible for the care of their apartment and preparation of their breakfast and lunch meals.

 

1324 Barbara Weit Rd
This location is a rural group home that offers four permanent beds; one accessible bedroom on the first floor and three bedrooms on the second floor. Though some residents attend day services Monday to Friday, there is various schedules of 24 hour staff supervision to this home through the week and weekends.

 

340 Court Street
This location offers four individual affordable and accessible apartment units on the upper level of 340 Court Street. The upper level is accessible by stairs and also by a lift system for persons in a wheel chair or with a mobility issue. There is also a laundry room located on the apartment level for the use of all residents.

Three of the apartment units offer a self-contained one-bedroom unit with a full kitchen/eating area, living room space, a separate bedroom, and an accessible bathroom. Persons with disabilities living in this housing possess a high level of independent living skills.

The fourth apartment unit is a three-bedroom apartment that provides housing to three persons that are dependent on a staff supported model of housing care. This apartment unit is a self-contained three bedroom unit with a full kitchen and dining area, living room space, three individual bedrooms, and an accessible bathroom.

 

328 & 330 Court Street

This location is our most recent that was built in 2019 and it’s a duplex style home.  One side has five bedrooms for residents and the other side has four bedrooms with one bedroom for respite for residents.  Both sides have 24-hour supervision daily.
